There are two black rings on the outer edges of the horizontal bar. Place one hand on the black ring and the other on the center of the bar, gently pull to adjust the width. Repeat on other side until desired width is achieved.

However, it can act as a guard rail, so long as the arm of the device peers over the mattress high enough (7 inches or more). The Stand-A-Roo Single Arm has a maximum height of 19 inches and should be placed alongside the torso of the user.
